Mafia: The Old Country features 22 collectible Charms, excluding those available at the start of the game or purchasable. These Charms are divided into four categories, each offering unique benefits to Enzo: Combat, Inventory, Health, and Loot. Additionally, there are Medallions, of which only one can be equipped at a time, and Beads, which can be attached to a bracelet. Once the bracelet is fully upgraded at Pasquale’s Store, you can equip up to three Beads, further enhancing your gameplay experience.
Charms can only be found within story chapters and are not in Exploration mode. They are typically located either near the starting area or shortly after cutscenes, making them easy to spot as you progress through the narrative.
Medallion Charms
There are five Medallion Charms to be found in Mafia: The Old Country. Four of them are hidden in the story chapters, and one can be bought at Pasquale’s shop.
San Bernardo da Corleone
In Chapter 7: Disgrazia, Enzo has to reach the forger in the basement of the Tannery. After the forger is found, turn around and search the forger’s area to find the San Bernardo da Corleone Charms Medallion.

San Cristoforo
In Chapter 10: La Festa, Enzo and Isabella arrive in the vibrant town of San Celeste. As they make their way toward the church, they notice the San Cristoforo Medallion tucked beside a door along the roadside.

-> Provides all of your guns with greater ammo capacity.
San Francesco D’Assisi
Buy the San Francesco D’Assisi Medallion in Francesco’s Store for 800 Dinari.
Effects:
-> Stealth Charm
-> Enhances your blade’s durability.
San Giuda
In Chapter 4: Il Barone, Enzo has to find Gennaro in Messina’s camp. This medallion charm is just a couple of steps away from Gennaro.
Effects:
-> Loot Charm
-> Guarantees that weapons looted from enemies will be full of ammo.
Sant’Agata
After working your first day in Chapter 1: Famiglia, Enzo meets Father Ciccone for the first time. He leaves the first Charm collectible in the game: San Celeste Statue with the Sant’Agata Charm.

-> Blesses you with a period of invulnerability when close to death.
Bead Charms
There are 23 Bead Charms to be found in Mafia: The Old Country. 18 are hidden in the story chapters, 5 can be bought at Pasquale’s Store.
Abile
In Chapter 6: La Difesa Siciliana, Enzo must free Giuseppe from prison. This bead charm is right in front of the prison cell.

Acuto
In Chapter 6: La Difesa Siciliana, Enzo must retrieve the printing plates from D’Amico’s Safe, which is in the top floor, the captain’s office at the San Celeste Town Hall. Here, you can find the Acuto Bead Charm.

-> Reduces the time needed to reach full accuracy when aiming.
Astuto
Chapter 7: Spirito Sportivo. At the chapter start, search the warehouse for the Astuto Bead Charm.

-> Grants you a higher chance of finding consumable items when looting.
Astuzia
Chapter 3: Palio. As Chapter 3 begins, Isabella wants to meet Enzo in the garden. Once she leaves, take a moment to explore the area. In the garden ruins, you’ll find the Beads Charm resting on the fireplace mantle.

Brutale
In Chapter 8: Industria, Enzo has to sneak into the factory at Collezolfo to reach the foreman’s office on the second floor. Right in front of the Foreman’s office door, on a wooden bench, is this Brutale Bead Charm collectible.

-> Enhances the damage dealt by your range weapons.
Caricato
In Chapter 5: Vendetta D’Onore, enter Ludovici’s Villa’s main building (Chapter V: Vendetta D’Onore) and go to the corridor. Look for a statuette on top of a table – that’s the Caricato Bead Charm.

-> Grants you a greater capacity of rifle ammo.
Collettore
In Chapter 8: Disgrazia, after L’Ombra escaped the Tannery with a car loaded with money, Enzo and Cesare chase after him. L’Ombra has an accident just south of Collezolfo. Here, behind a wooden cross, is this bead charm collectible.

Destrezza
In Chapter 13: Capiregime, Enzo sneaks into an abandoned village to kill Mazzone in the Abandoned Village. In the area where Enzo finds Mazzone, there is also this Destrezza Bead Charm.

-> Speeds up your reloads with all guns.
Economico
In Chapter 11: Mattanza, Don Torrisi, Luca, Cesare, and Enzo drive to Don Galante’s tuna factory northwest of Porto Almaro for a peace talk. Here, near the entrance, is the Economico bead charm.

-> Ensures you will find more ammo when looting.
Esplosivo
Buy the Munito Bead in Francesco’s Store for 600 Dinari.
Effects:
-> Inventory Charm
-> Helps you carry a greater number of grenades.
Fantasma
In Chapter 4: Il Barone, Enzo prevents Isabella from being captured by Messina. He follows Messina to his camp. At the starting spot, right after the cutscene, you can find the Fantasma Bead Charm.
Effects:

-> Dampens the sound of your footsteps
Fortezza
Buy the Fortezza Bead in Francesco’s Store for 1200 Dinari.
Effects:
-> Health Charm
-> Offers you some protection against harm during duels.
Fortunato
Chapter 2: Palio. Enzo and Luca set out on horseback, heading toward a race just north of the town of San Celeste. There, on a wooden platform, rests the Fortunato Beads Charm – a symbol of fortune and intrigue.

Ingegnoso
In Chapter 3: Pizzu, after retrieving the whiskey in the Porto Almaro impound yard, Enzo and Cesare end up cleaning out an Abandoned Farm – it’s where Enzo gets to shoot for the first time. Here, inside the main building, is the Ingegnoso Charm Bead.

Medicale
In Chapter 7: Spirito Sportivo, Enzo can enter the Bordello in San Celeste and reach a balcony on the top floor. From there, he can cross over to another room that holds the Medicale Bead Charm.

Paziente
In Chapter 12: La Forza del Destino, Enzo travels to Palermo and crosses the sewers to get to the opera house. After climbing a ladder up from the sewers, he exits into a small storage area. Here is the Paziente Charm Bead.

Perseveranza
In Chapter 9: Industria, Enzo has to enter Lorenzetti’s office in Collezolfo. Do so by climbing up the balcony at the courtyard and entering through the green balcony door. In the first room is the Perserveranza Bead Charm.

Pragmatico
In Chapter 9: Industria, Enzo must infiltrate the factory. At the factory’s first section, just before passing through the metal gate that splits the building, there’s a table. On it, you’ll find the Pragmatico bead.

-> Improves your accuracy when blind firing or shooting from the hip.

Resistenza
In Chapter 12: La Forza del Destino, Enzo travels to Palermo. During the “Follow the priest” section, Enzo has to follow a small stream to open a gate from the other side. He will encounter two skeletons. One of them died because of a cave-in. Nerby is the Resistenza Bead Charm.

-> Boosts how much you recover when close to death.
Valore
In Chapter 11: Mattanza, fight until you have to climb a ladder to shoot the sniper in a big open area. After the shootout, Leo will go through a big, blue door to the next shooting segment. Right after passing through the blue door, turn right to find this Bead Charm on top of a workbench.

-> Reduces the blur effect caused by incoming gunfire.
